To register as a York Region resident, complete and submit the Application Package below. You may also call us at 1-888-762-2950 to request a hard copy. There is no registration fee.

Eligibility Appeal Process Brochure

If your application for Mobility On-Request Paratransit service has been denied based on eligibility, the Eligibility Appeal Process offers you the opportunity to appeal the decision. You may appeal a decision regarding eligibility, level of eligibility or type of assistance required.

Notice of Appeal Form

Complete this form if your Mobility On-Request Paratransit application is denied. Upon receipt of your completed Notice of Appeal form, your original Mobility On-Request Paratransit application and form will be sent to the Appeal Panel. You will then be advised of your hearing date.

Support Person Assistance Card Application Form

If you need to be accompanied by a support person when using York Region Transit (YRT), you can apply for a Support Person Assistance Card. The card permits a support person to travel on YRT with the eligible customer (cardholder) at no additional cost.
